PARISH REGISTRATION FORMS

Registration forms for new parishioners and information about faith formation classes for children and teens, youth ministry, and sacramental preparation classes for First Reconciliation and First Eucharist for the 2016-2017 school year are now available from the parish office or can be downloaded by clicking below.

The registration forms can be downloaded to your computer and completed using Adobe. Forms requiring signatures can be mailed, dropped off at the parish office or scanned. All forms should be sent to secretary@stbernadette.org
